As an Internist/Pulmonologist, I had the distinct privilege of working under Doug McMillan’s leadership for 28 years. In that time, I saw a small community hospital with a medical staff of 15 evolve into a regional healthcare provider with over 70 primary & subspecialty providers.
His vision for Cody Regional Health resulted in a new, state-of-the-art Emergency & Radiology Departments, a Cancer Treatment Center, a free-standing Hospice facility, Wound Care Clinic, a medical office complex providing space for a variety of outpatient clinics to include a Walk in Clinic, Internal Medicine, Psychiatric, Orthopedics, General Surgery, Cardiology & OB/Gyn. Most recently, three outreach clinics were launched in neighboring communities, as well as a helicopter flight service in collaboration with a level one trauma center north of Cody.
Qualities that come to mind when I think of Doug are integrity, honesty, & compassion which foster the deep respect I have for this man. Doug’s personal values and qualities are above reproach, and it is without hesitation that I recommend Doug to any organization seeking his guidance and leadership.

I have known Doug McMillan for the past 28 years, working with him as a Medical Staff leader and more recently part of Senior Leadership in the role of CMO. I have seen our Medical Staff more than double during that time, bringing on new service lines and expanding existing ones to fit the needs of our patient population. Doug has been instrumental in recruiting new physicians, enlisting the help of reputable recruiting firms that have helped over the years and taking the time to personally reach out to candidates, encouraging them and their families to visit and see what our community has to offer. His interest in them and encouragement has helped to build our Medical Staff to what it is today.
The Hospital has grown from a $20 million annual net revenue CAH to a health system with over $100 million dollars annual net revenue. The physical plant has been consistently updated, remodeled, and expanded to meet the ever changing landscape of Medicine. We have continued to expand services while keeping our focus on the patient. It is a tribute to Doug's operational leadership that we continue to change and evolve, and remain profitable.
I have enjoyed working with Doug and respect him and his commitment to the hospital and the community. He has exhibited exceptional leadership abilities throughout his time here. He has developed many friendships and contacts with colleagues throughout this region which I feel is an asset in the daily running of a hospital system. I have had the privilege to work with Doug in his role as CEO of Cody Regional Health for over 20 years. During that time, we were able to work together to create and implement a facility wide master plan in multiple phases with multiple funding sources. The result was a totally transformed campus that faced the community as a true community hospital that also enhanced the vitality of the community economically. We have often referred to it as a “Critical Access Hospital on Steroids” because of the continued growth and quality of care provided. While new facilities certainly helped with this growth, having worked in similar facilities for 40 years, I know that the leadership at the top is what allows the success to happen every day over a long period of time.
I watched Doug build a strong leadership team with growth in physician services while maintaining financial success. Decisions were visionary but prudent as Doug and his team added services to serve the community while maintaining a strong financial position. We watched as Doug worked across all the stakeholder groups including the community, the board, physicians, and staff to find consensus on answers and decisions to move projects and operations forward. With Cody Regional Health being a taxing district, Doug and his leadership team had to be sensitive to the community for providing value for those tax dollars. During his time, CRH was successful in mobilizing the community to support tax increases to fund the improvements.
Working with over 200CAH over the years, I would highly recommend Doug to help an organization move forward. Over the years, I have appreciated the lessons I learned about leadership and the healthcare world from Doug. Because of all of these skill set, I will always be ready to support him in the future.

I had the pleasure of working with Doug for just over five years. I would have stayed in Wyoming working under Doug's leadership if I had not pursued my own career path to becoming a rural health CEO like Doug. In fact, he was my inspiration to pursue this direction. I purposely emulate leadership behaviors that I had learned from him. For example, learning and knowing all staff by their names and a little something about them personally and/or about their families. Doug's uncanny ability to remember so many people and details is impressive. Just this one small but meaningful leadership act has been recognized by my new team here in Missouri. As I tour physician candidates through my hospital campus and as I acknowledge staff personally, this has influenced candidates to choose our hospital over others......a gift I learned from Doug.
In fact as I conduct my own physician recruitment activities, I think of Doug and his style of personally engaging with candidates and conducting pre-interview phone conversations with them. Doug would include me in many of these calls so I witnessed his ability to quickly put people at ease and engage them in free-flowing conversations which helped us evaluate an initial fit into our culture and the remote environment of Cody.
Doug is inclusive and utilizes his senior team as consultants and counsel for his decision making. He is humble and very open to new ideas, supportive of change and appreciates different opinions and perspectives.
I also had the privilege of serving through the Covid pandemic under Doug's leadership. This was uncharted territory and he handled it with courage, confidence, and grace even in the worst of times such as when we lost our own staff to Covid and when answers were needed but there was not a source of truth to get those answers. He led with grace under fire and with conviction to always place people first in making decisions.
I think it is his people-centered leadership style that I respect the most. Not always is healthcare led in a manner that doesn't put finances above the human aspect. I witnessed his people-centered approach in action when we had to do a reduction in force early in my time at Cody Regional Health. The thoughtfulness of how people were managed and treated respectfully and transparently during one of the most difficult times of their lives being impacted was admirable. In a very difficult time, Doug's leadership style and deeply held values around the importance of people was what got us through successfully and with staff appreciation even in light of termination of their positions.
I think you can tell that I admire Doug's leadership skills, style, and effectiveness. I do try to emulate his leadership because I believe that makes me a more effective leader for the hospitals I support.

I appreciate Doug’s ability to join a prospective candidate with the right organization. As my family and I searched for a new location, we found ourselves taking a trip to Wyoming where Doug interviewed me for a leadership position. Conversation prior to my arrival and during my stay allowed my family to engage with the community as well as many members of the organization. Doug was clearly seeking an individual that would fit the organization as well as the community. He casually asked questions in an effort to understand who I was and who my family was. Building a relationship was important to the process and more important to Doug himself.
